        Mon 8:00 am -5:00 pm
        Tues 8:00 am -5:00 pm
        Wed 9:00 am – 5:30 pm
        Thurs 8:00 am – 5:30 pm
        Fri 8:00 am – 4:00 pm.
      • For an appointment in Queens, please call (718) 204-0548. The Queens office is open Monday through Friday, 9am to 5pm. Our staff will make every effort to schedule your appointment as soon as possible.
      • If you are unable to keep your scheduled appointment, please give us at least 24 hours notice.
      • Since we do treat trauma, occasionally one of our patients may have an orthopedic emergency that requires immediate surgical attention. Although this happens rarely, unfortunately, at these times, we may need to reschedule your appointment.
      • In the event that you need emergency care, one of our physicians will always be available.

      If this is your first time visiting our office, or if you have not been seen by one of our physicians for more than 2 years, you will need to complete our patient demographic, history and HIPAA forms.

      If you choose to complete the forms in the office YOU MUST ARRIVE 20 MINUTES PRIOR TO YOUR SCHEDULED APPOINTMENT TIME in order to be keep your scheduled appointment time.

      If you are involved in a Workers Compensation claim or a No Fault claim (motor vehicle accident), please complete the appropriate additional forms:

      What to bring to your appointment

      • All of your health insurance cards
      • Physician referrals
      • If applicable, please bring prior X-rays, MRI studies, or previous operative notes.
      • Workers Compensation / No Fault claims: Please bring all appropriate paper work to prevent delays.
